Consent One

I hereby give my informed consent to participate in club sports activities offered by The Orange County Track & Field Club (hereinafter referred to as “the Club”) located in Orange County, California.

Consent Two

I acknowledge that participation in club sports involves inherent risks and hazards, which include but are not limited to physical injury, illness, and death. I understand that these risks cannot be eliminated, even with the Club’s safety measures and precautions.

Consent Three

In consideration of being permitted to participate in club sports activities, I hereby release, waive, and discharge the Club, its officers, employees, agents, and volunteers from any and all claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or my property, whether caused by the negligence of the Club or otherwise.

Consent Four

I acknowledge that I have read this document and fully understand its contents.

Consent Five

I agree to assume all risks associated with participation in club sports activities and acknowledge that the Club has not made any warranties or representations regarding my safety.

Consent Six

I further acknowledge that I have been given the opportunity to seek independent legal advice before signing this document.

Consent Seven

I hereby authorize the Club to provide me with emergency medical care, transportation, and treatment in the event of injury or illness that may occur during my participation in club sports activities.

Consent Eight

I also agree to comply with all rules and regulations of the Club, including but not limited to safety rules, and I understand that failure to comply with these rules may result in my expulsion from the Club.

This permission and release of liability shall be binding upon my heirs, executors, administrators, and assigns.
